By submitting this application, I confirm that the information provided is accurate and complete. I understandand agree to the following:

1. Membership & Subscriptions 

○      I apply to become a member of Curve Club

○      I authorise Curve Club to collect membership subscription payments by the method I provide.

○      I acknowledge that subscriptions may be set up on a recurring basis (e.g. monthly/annually) until I cancel in writing, subject to the Club’s cancellation policy.

2. Terms & Conditions
   

○      I agree to abide by the Club’s rules, policies, and code of conduct, as updated from time to time.

○      I understand that membership is subject to approval and may be revoked if I breach these terms.

3. Data Protection     

○      I consent to the collection, storage, and processing of my personal data for the purpose of administering my membership and subscriptions, in accordance with applicable data protection laws.

○      I understand that I may request access to, correction of, or deletion of my personal data at any time by contacting the Club.

4. Cancellation Rights  

○      I understand that I may cancel my membership by providing written notice in accordance with the Club’s cancellation policy.

○      Refunds of subscription fees, if any, will be processed in line with that policy.
